- Initiate assessment of Client’s care needs and implement personalized care and goals.
- Monitor Client’s progress and adjust care plan as needed.
- Facilitate Client and care giver education.
- Communicate Client’s care needs identified with Client’s Responsible Party, Physician and other key care members.
- Assist with scheduling Client’s medical appointments.
- Assist with Client’s transportation to medical appointments.
- Provide family support resources, including educational materials, counseling, and access to support groups.
